The BASC-3 Flex Monitor tool provides the ability to monitor and track behavior over a period of time. This tool can be used to develop effective progress monitoring forms that can be used to track changes in behavioral and emotional functioning.
Click the More Actions button from the Examinee (Home) page.
Click the Manage BASC-3 Flex Forms link
This will take you to the Manage BASC-3 Flex Monitor Forms page.

| Create New Form |
Clicking this button will open the Form Builder page to build a new form. |
| Open |
Clicking this button will open the selected form in the Form Builder page. Forms with an ACTIVE status will be in VIEW ONLY mode. Forms that are in DRAFT status are editable. |
| Create A Copy |
Clicking this button will create a copy of a selected form and will open the form in the Form Builder page. |
| Change Status |
Clicking this button will change the status of a custom ACTIVE form to INACTIVE. It will also change the status of a custom INACTIVE form to ACTIVE. Status cannot be changed for a form in DRAFT mode. All forms with an ACTIVE status, will appear in the list of assessments available to assign to an examinee. Forms with an INACTIVE status will not appear in the list. Forms that are created by the system (system generated forms) cannot be inactivated. |
| Generate PDF |
Clicking this button will allow the user to generate a PDF version of the form. User can select between English and Spanish versions for each form. |
