Click the “Show item detail” toggle to include display of elapsed time, drop-down entry fields, trial checkboxes, comments, and caregiver question checkboxes for items where applicable.

When performing item score entry, enter the score for each administered item. If item scores are entered, the system calculates the raw score and the raw score will appear in the report, but not on the Subtests tab on the Assessment Details page.

A score must be filled in for all items between the basal and discontinue point for each administered subtest. The subtest is considered unscorable until all scores between the basal and discontinue have been filled in.

For more details about item score entry, age-based start points, basal and reversal rules, and discontinue rules, please see the Bayley-4 Administration Manual or record form.
