For the Bayley-4 Cognitive, Language, and Motor Scales, there is an Overview tab, a Subtests tab where examinee item scores or the subtest raw score(s) may be entered, and a Behavior Observation Inventory tab.

Reason for Referral |
The reason the child was referred for testing.

Adjustment for Prematurity |
Required. Indicate whether the child’s age should be adjusted for prematurity. If 'Yes' is selected, the child’s expected birth date must be entered and adjusted test age will be used to determine the subtest start points and to score the test.

Expected Birth Date |
This field appears and is required if 'Yes' is selected for the Adjustment for Prematurity field. The expected birth date is the date the child was originally expected to be born. The expected birth date should be later than the child’s chronological date of birth.

The Bayley-4 allows raw score entry or item score entry. For each subtest, enter either the raw score OR subtest item scores.

Raw Score Entry
To perform raw score entry, enter a valid raw score for the administered subtest(s).
Item Score Entry
To perform item score entry, click the item entry toggle to switch to item entry mode. Enter item scores in the single-character auto-advance item entry boxes. Click the “Show item detail” toggle to include display of elapsed time, drop-down entry fields, trial checkboxes, comments, and caregiver question checkboxes for items where applicable.
When performing item score entry, enter the score for each administered item. If item scores are entered, the system calculates the raw score and the raw score will appear in the report, but not on the Subtests tab on the Assessment Details page.
A score must be filled in for all items between the basal and discontinue point for each administered subtest. The subtest is considered unscorable until all scores between the basal and discontinue have been filled in.

If you completed the Behavior Observation Inventory portion of the Bayley-4 assessment, you may enter the responses so that this information will be included in the Bayley-4 Cognitive, Language, and Motor Scales Score Report.
