The name of the teacher who completed the questionnaire about the examinee. |
Once you start entering raw scores in a section, you need to complete the entire section. If you did not administer all of the items, enter a zero (0) in the associated Raw Score field.
When
you click the Save button to save the assessment, the system
checks to ensure all of the required data have been entered. If any
data are missing or invalid, you will be asked if you still want to
save the assessment. If you choose Continue
and Save, the record will be saved with a status of “Enter Responses”
or “Needs Editing,” depending on the data still necessary to score
the assessment. When you open the assessment record again, a message
will be displayed indicating what is missing or invalid.
If you choose Make Corrections,
you are returned to the page where you can finish entering the data.
The Questionnaires section of the page is not required.
