For EMS, there are four reports available:
EMS Individual Report -Self-Report Form
EMS Individual Report - Observer Form
EMS Multi-Rater Report
EMS Progress Report
Click Select next to the report name to select that report. Inventory needed and usages available are displayed with each report. Multi-Rater and Progress reports are free and do not require usages.
The EMS Self-Report and Observer rating forms contain relevant demographics, the EMS Score Profile, EMS Score Descriptive Classifications, Critical Items, and Item Response Table sections.
The checkboxes and radio buttons under Report Selection and Configurations allow the user to choose the report file type and report options for the Self-Report and Observer rating forms.

| Include Item Responses as Administered | Select to include the Item Responses by numeric value. |
