Entering WMS-5 Scores

The Wechsler Memory Scale® (5th ed.; WMS®-5; Wechsler, 2025) is the most widely used scale for assessing memory for individuals ages 16:0 through 90:11. It is an individually administered measure of learning and memory processes for the identification, diagnosis, and follow-up evaluation of memory difficulties. There are three WMS-5 forms available for flexibility of administration (i.e., Standard, Short, and Alternate). Each form includes two age-based batteries: a battery for individuals ages 16-69 and a battery for individuals ages 70-90.

The WMS-5 on Q-global includes manual entry with an Overview tab and a Subtests tab. The Overview tab includes administration settings and assessment demographics. The Subtests tab allows for a maximum of 10 WMS-5 subtests to be entered, plus a BCSE score and two CVLT 3 scores.
