The WRAT5 provides an accurate and easy-to-administer way to assess and monitor the reading, spelling, and math skills in people ages 5–85+ and helps identify possible learning disabilities.
For WRAT5, there is a Demographics tab and Raw Scores tab.
There are two (2) WRAT5 forms;
WRAT5 Blue Form
WRAT5 Green Form
The Demograhics and Raw Scores tabs are the same for each form.
Field Name |

Current (or Most Recent) Grade Level | The grade level of the individual at the time of the assessment
administration. The initial default is blank or the previously
entered/selected value if a previously saved assessment is being
viewed. The Grade Level can be selected from the pull-down list.
Adults are automatically assigned to grade 12+ and only age norms are available for reporting for adults 20+. |

composite score
is based on the sum of the Word Reading and Sentence Comprehension standard
scores.
The
Reading composite is not provided for kindergartners or 5-year-olds.
