Parent Rating Forms | |
Rater's first name | The first name of the parent or guardian who completed the rating form. |
Rater's last name | The last name of the parent or guardian who completed the rating form. |
Relationship to child | The relationship of the parent or guardian to the examinee. If ‘Other’ is selected, a text box is displayed to collect additional information. |
Teacher Rating Forms | |
Rater's first name | The first name of the teacher who completed the rating form. |
Rater's last name | The last name of the teacher who completed the rating form. |
Position |
The teacher’s job title. If ‘Other’ is selected, a text box is displayed to collect additional information. |
Time known child | The length of time the teacher has known the examinee. |

When you click the Proceed to Reports or Save updates button the assessment is saved and the system checks to ensure all the required data has been entered. If any required data is missing, the record will be saved and a message will be displayed at the top of the screen describing the condition(s) that must be resolved before a report can be generated for the record.
