For PPVT-4, two reports are available: Individual Score Summary Report and Individual Progress Report.

The PPVT-4 Individual Score Report contains four sections: Score Summary, Narrative Report (optional), Diagnostic Item Analysis (optional), and Suggested Interventions (optional).
Score Summary
The score summary contains a distribution curve with standard deviations, standard scores, percentile ranks, stanines, and descriptive categories. The standard score, percentile rank, stanine, and confidence interval are plotted.
Narrative Report
The optional narrative report page contains PPVT–4 narrative information, general overview of the PPVT–4 assessment,
examinee demographic data and results for the examinee’s performance on the PPVT–4.
Diagnostic Item Analysis
The Individual Diagnostic Analysis is only available when item entry is used.
The Diagnostic Analysis table presents the PPVT–4 item numbers and columns for the Noun, Verb, and Attribute parts of speech that an item may measure. If an item is a scale item for a specific part of speech, the box is left white. If an item is not a scale item for a specific part of speech, the box is shaded. If an item has not been answered correctly, the error is identified with an 'X'. Basal and ceiling items are identified with an asterisk (*).
The part of speech name, number of items attempted, number of items correct, percent correct, and a bar graph visually showing the percent correct is also presented.
Suggested Interventions
The optional suggested interventions section contains intervention suggestions based on the examinee’s PPVT–4 standard score. The Suggested Interventions section includes vocabulary principle descriptions, The Bridge of Vocabulary suggested intervention activities, and activity descriptions and references.

The Individual Progress Report can be printed for Form A and/or Form B of the assessment. The test administrations are organized by date (earliest test first, most recent test last). The Individual Progress Report contains Growth Scale Value (GSV) information, a GSV chart, a PPVT-4 Administration table (test date, norm selection, test/form, standard score, GSV, mean and range of national GSV scores), and General PPVT–4 test-retest suggestions.
