Sensory Profile™ 2 has five forms on Q-global: Infant Sensory Profile™ 2, Toddler Sensory Profile™ 2, Child Sensory Profile™ 2, Short Sensory Profile™ 2 and School Companion Sensory Profile™ 2.
The Sensory Profile™ 2 family of products aids professionals in measuring an individual's sensory processing abilities and profiling the effect of sensory processing on functional performance in an individual’s daily life.
• Infant Sensory Profile™ 2 is a caregiver questionnaire designed for children ages birth through 6 months. The caregiver questionnaire consists of 25
items and is available in English and Spanish. Raw scores, cut scores, and percentile bands are available for one scale.
• Toddler Sensory Profile™ 2 is a caregiver questionnaire designed for children ages 7 through 35 months. The caregiver questionnaire consists of 54 items
and is available in English and Spanish. Raw scores, cut scores, and percentile ranks are available for seven sections and four quadrants.
• Child Sensory Profile™ 2 is a caregiver questionnaire that consists of 86 items. Child Sensory Profile 2 can be administered to children ages 3 years 0
months (3:0) to 14 years 11 months (14:11). The questionnaire is available in English and Spanish. Raw scores, cut scores, and percentile bands are
available for nine sections and four quadrants.
• School Companion Sensory Profile™ 2 is a 44 item teacher questionnaire intended to be used for students, ages 3 years 0 months (3:0) to 14 years 11
months (14:11). Raw scores, cut scores, and percentile ranks are available for five sections, four quadrants, and four school factors.
• Short Sensory Profile™ 2 is a caregiver questionnaire that consists of 34 of the most discriminatory items from Child Sensory Profile 2. Short Sensory
Profile 2 can be administered to children ages 3 years 0 months (3:0) to 14 years 11 months (14:11) and is designed to provide quick information for
screening and research programs. The questionnaire is available in English and Spanish. Raw scores, cut scores, and percentile bands are available for
four quadrants and two sections.
All Sensory Profile 2 forms can be manually entered or administered through On-Screen Administration or Remote On-Screen Administration.
Sensory Profile 2 has three tabs: Overview, Item Entry, and Assessment and Planning Demographics. The Short Form only has two tabs: Overview and Item Entry.

The Adolescent/Adult Sensory Profile is a 60 item self-report questionnaire most appropriate for individuals 11-65+ years of age. This form can be manually entered or administered through On-Screen Administration or Remote On-Screen Administration. It is summarized using four quadrant scores.
